• You are requisite to complete the registration form. Must make sure that the information provided is correct. Because this information will be use further in Entry Test registration as well as for admission.
• Login ID will be given by the system automatically, you just have to give your own password.
• Remember and safe your password for future use. You will have option to change password at any stage.

Note: Candidates for the test center Gujrāt asked to report to the campus HAFIZ HAYAT to submit registration application.
Instructions for completing the form and print the data online
1. If any Download Manager is installed in your computer then disable the Download Manager integration with your browser or delete it.
2. If you have made any mistake in data entry, complete the new form from the beginning, to go to the home page. You can fill in and print out a new form with same CNIC, until you got to admit card.

Attachments with the Entry Test Form

• Photocopy of CNIC or Computerized B-Form issued by NADRA.
• Photocopy of Father’s CNIC.
• Two Passport size photographs.
• Paid Challan Form Receipt No. 1.

Submission of Entry Test Form

• Candidates should have to submit Entry Test Form in the working hours (Monday to Friday 08:00 AM to 01:00 PM), but make sure before submission that the forms are forms are completely and correctly filled.
• It is not vital for an applicant to appear in person for submission to Entry Test Form.
• Incomplete Entry Test forms or sent through Post/Courier are not acceptable.
• The Entry Test Fee Rs. 350/- may be deposited on printed challan in any Online HBL Branch in Pakistan.

Requirement of Photo-ID at the Test Centre during Entry Test

Applicants are instructed to bring photo-ID like CNIC, SSC Certificate, Domicile etc. for presentation in the Entry Test centers during the Entry Test.
